Physicians have been using external fixation to treat fractures for more than 2000 years, aiming to immobilize the fracture while preserving soft tissue integrity. The primary goal of external fixation is to maintain the length, alignment, and rotation of the fracture. It can serve as provisional or definitive fixation, and can be performed in conjunction with partial internal fixation if necessary.
Fracture healing physiology depends on the mode of fixation and stability. Relative fracture stability, such as that provided by external fixation, results in secondary enchondral bone healing. Various construct alterations can adjust the stability of the fracture as required by the clinical situation.
